Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
Payment is due at the time of treatment. If you need to make payment arrangements, please speak to a member of our front desk team before your appointment. If you do not have insurance or need to cover the costs of your co-payment or out-of-pocket expenses, our office accepts cash, personal checks, MasterCard, Visa, Discover, and American Express for your convenience. We have also partnered with CareCredit to provide you with other financing opportunities. You can learn more about this service and complete your application in the link below. Dental Insurance
We make paying for your dental care as simple as possible. Our office participates with Delta Dental Premier and PPO insurance plans. If we are out of network with your insurance plan, we will still file claims on your behalf so that you can get the most out of your coverage.
